Call us at (765) 459-8000 to schedule your hormone consult with Dr. Haendiges!

• Download and print off the Hormone Consult Packet.  
  • IMPORTANT: You must print and complete these forms PRIOR to your scheduled appointment with us.  Please bring the completed forms to your scheduled visit. It is essential for Dr. Haendiges to have this information at the time of your consult.  
  • Please schedule this appointment separate from your normal yearly exam due to the extra time it requires. 
• At your consult, Dr. Haendiges will provide you with all of the tools you need to make an informed decision about your hormone health.
• You will take your Hormone Profile Test home with you after the consult.
• We will provide you with detailed instructions so that you may collect the required samples, which you will then mail to the lab.
• You will schedule a follow-up appointment with Dr. Haendiges to discuss your results (approximately 10 days after your initial consult ).
• Dr. Haendiges will present you with all available treatment and/or therapy options.
