We would like to take this opportunity to thank you for choosing Haendiges and Associates as your Women's Healthcare Provider. Be assured that your first visit with us will be a pleasant and rewarding experience.

Please bring with you the completed New Patient Forms.  The forms should be completed front and back in ink, as this becomes part of your permanent record.  If you have insurance, please bring your insurance card and any other information we may need to file with your insurance carrier.  If your insurance requires referrals to specialists, please be sure to call your Primary Care Physician to obtain a referral to our office prior to your appointment.

All payments are due at time of service.  As a courtesy, we are happy to submit insurance claims for you.  However, if payment is not received within 60 days, it is your responsibility to pay the balance due in full.  We accept cash, check and credit cards.
